Youth LOCK-IN @WOFCC
INFORMATION
Please fill out all of the information below.   You are reserving a spot for your child to participate in this event by filling out this registration form.  All funds are due upon check-in.
By completing this form, you (guardian/parent) and your youth(s)/ participant(s) understand that you are reserving a spot for the
Youth LOCK-IN @WOFCC. 
The event is set to take place on
Friday, April 13, 2018, 8PM
until Saturday, April 14, 2018, 8AM.  

Early sign-up is from
Thursday, April 5th, 2018
through Sunday, April 8, 2018. 
All guardians/parents who have signed up any youths/participants in this time frame will be required to pay
$10 per youth/participant
up to 2 youths/participants,
and $5 per youth/participant afterwards.

Regular sign-up is from Monday, April 9, 2018 through checkin on April 13, 2018. 
All guardians/parents who have signed up any youths/participants in this time frame will be required to pay
$20 per youth/participant
up to 2 youths/participants, and
$10 per youth/participant afterwards.  

All funds are to be remitted upon
check-in on April 13, 2018. 
Completing this sign-up form notifies us that we can anticipate the attendance of your youth(s)/participant(s).  Which, in turn, allows us to ensure that we are maximizing the experience.  

More information will be given to each guardian/parent upon
completing all requested documents
on the night of the
Youth LOCK-IN @WOFCC. 
All profits from the
Youth LOCK-IN @WOFCC go to the
WOFCC CHAMPIONs Scholarship Fund.
